Pretty much the same thing that causes regular cancer; a mutation in the DNA of the cell which causes it to grow out of control.
A mutation like that can be caused by the cells just being old and having divided a lot, the cells being disrupted by radiation… Thankfully it’s not a hugely common cancer.
